Your social prescribing link worker can talk to you about a problem you have or a change you would like to make.  Your link worker will not only give advice and encouragement, they will also connect you with suitable local services  to support you.

Dionne Hayter is our Social Prescriber via the Beckenham PCN (Primary Care Network) she is available to help with the following:

Money worries, loss of a loved one, unhappy where you live, problems finding work, making healthy lifestyle changes, feeling lonely, living with a long term health condition.

You can see Dionne by booking an appointment via reception where they will complete a short referral form, you do not need to see a GP to be referred.
